How To Use the CTWUG Requisition Forms

What Are They For?

Requisition forms are to be used whenever CTWUG pays money - whether to a wugger or a vendor or company. It helps the CTWUG Committee keep track of money paid out and what invoices are outstanding. They are also a record of authorisation for spending by the commitee.

There are two different forms.

• Refund Requisition Form

The Refund Requisition Form is filled in by anyone who has spent money and is requesting CTWUG to reimburse them. Please get committee approval BEFORE spending money as if it was not pre-approved it will not be refunded.

• Prepayment Requisition Form

The Prepayment Requisition Form is used in cases where the amount of money is larger and the person making the purchase may need the money upfront.

These forms don't differ in how they are filled in, but they do differ in how money is disbursed.

How Do I Fill It In?

First, obtain a copy of the relevant form from the CTWUG Treasurer. Contact them here.

Open the file. It should look like this:

In the first section, enter the date, fill in your name and enter the name of the person or company that the money should be paid to.

In the main block, fill in the details for the items the money was for as well as the amounts. If it is for more than one item type, list them separately.

In the Account details block, fill in the bank account details of the person or company the money should be paid to.

The remainder of the blocks are for the committee's use and you don't need to fill them in.

Submit the form to the CTUWG Treasurer with the necessary invoice. Send the form and invoice here.

NO MONEY WILL BE PAID OUT WITHOUT THE INVOICE BEING SUBMITTED. IF YOU WOULD LIKE A REFUND FOR MONEY SPENT, DO NOT LOSE THE INVOICE.

A Note About Prepayments

Prepayments are very carefully controlled and may often be declined in favour of refunds.

Since when requesting a Prepayment Requisition, you won't have an invoice, you don't need to submit it with the requisition form.

Once the money has been paid, however, you need to submit an invoice to CTWUG within 30 days or you will be required to pay back the money prepaid to you by CTWUG.
